A company with an IT system that is inadequate and shows my policy as cancelled. I get an email confirming payment for the policy together with a letter saying that I need to make a payment and if I do not then the policy will be cancelled and I will be in debt!

I really appreciated talking to real people rather than some anonymous computer based system in setting up the account and recipients. When there was a problem with me entering a wrong account number they called and I was able to correct it over the phone.
